You must be in the Lanyard Discord server for this to work. You'll also need your Discord User ID (enable Developer Mode in Discord settings and right-click your profile).
1. Join the Lanyard Discord server
2. Get your Discord User ID (Developer Mode → right-click profile → copy ID)
3. Fill out the form and create your page
4. Save your edit key to modify your page later
